Skilled Nursing Service


Hazel I. Findlay Country Manor (HIFCM) employs Registered and Licensed Practical Nurses that provide 24 hour nursing care for 150 residents.  They are assisted by State Certified Resident Care Aides and consultant therapists who are under the direction of the resident’s physician.  HIFCM accepts Medicare and Medicaid, private pay, as well as third party insurances.

The Skilled Nursing Services at Hazel I. Findlay Country Manor are designed for residents who are in need of 24 hour care and rehabilitation services to assist them to rehab after an acute or short-term illness.  The skilled level of care provides the regular services of a registered nurse for treatments and procedures and also services provided by specially trained professionals such as physical and occupational therapists.  With a resident’s progress, immediately following the treatment a discharge plan will be implemented to assist a resident in returning home and/or to a lesser level of care.

HIFCM’s nursing department is supervised by a Registered Nurse who serves as the Director of Nursing.  Under the Director of Nursing the staff of Registered and Licensed Practical Nurses and State Certified Resident Care Aides provide 24 hour nursing care.  In order to maintain our concept of personalized care, three smaller units are supervised by Assistant Directors of Nursing.  These Unit Managers are the primary contact person for family members and loved ones at the facility.
